Abstract

Objectives : The purposes of this study were to assess the health problems of young children from birth to age 2 years old in a rural community by the cross-sectional study method and to test the feasibility of this study method for maternal and child health data collection in rural area.

Methods : A questionnaire survey was conducted for 244 parents or the main caretakers of the young children by trained nurses and physical examination, pathologic examination and developmental test by a pediatrician for 244 young children, all of 2 years (25 months to 36 months) old children in Chung-Song County between January 3 to February 15, 1998.

Results : 1. Maternal characteristics and pregnancy outcome. There was no mother who was younger than 20 years old but mother of 35 years old and above accounted for 8.2%. Mothers who had 5 or more pregnancies accounted for 10.2% and 42.2% had history of 1 or more induced abortions. Cesarean section rate was 23.4%; multiple birth rate, 1.2%; low birth weight incidence rate, 4.1%; premature delivery rate, 4.5%; and babies with congenital anomalies, 10.6%. 2. Child feeding, vaccination and process of development. Proportion of children who were breastfed for 4 months or longer was 40.2% but weaning time and method were inappropriate for most of the children. Immunization rates for childhood routine vaccinations were over 90% except measles (89.3% at 9 months after birth) and time of immuruzation was mostly on schedule. There was no child who showed developmental delay in motor and language. Proportion of children who were tested for visual and auditory functions test was 5% and 12.7% of the child caretakers were other than mother. 3. Health, growth and development status of young children. There was no child whose body weight was under 3 percentile for age including all the children who were born with low birth weight. Three children were found to have both systolic and diastolic blood pressure higher than 95 percentile of American children¡¯s blood pressure. Dental caries was found in 33.3% of all children and 2.9% of them had anemia (hematocrit ≶ 34). In urinalysis, pyuria was found in 1%. In Denver Developmental Screening Test, 98.4% of children were normal, 2 children(O.8%) and were found to be delayed.

Conclusions : The data obtained by interview in this study should have been collated with the medical record to test the reliability but the medical records were not available. However, it seems that a crosssectional survey can be used for quick longitudinal data collection for maternal and child health. The findings of this survey suggest that the health of children in this rural area is not poorer than the general child population of Korea.
